Our Policies

Procedures:

Please follow this procedure for all orders.  Pour models in orthodontic stone.  Check for accuracy before sending the case.  Send bite registrations for splints, biteplates, habit appliances as wll as some functionals.  Please allow for some possible breakage of teeth during construction of a small percentage of appliances.  For most appliances allow for 5 days in lab.

Billing and Payment: 

Invoices:  Invoices are generated for each patient's prescribed appliance on the invoice provided.  Invoices will be dated when received and once again when the work is completed.  Please retain invoices for your records and future reference.

Monthly Statements:  Monthly statements are sent with all transactions that have occurred on your account for the current month.  Statements are sent on the last working day of the month.

Past due accounts:  Payment terms are net 15 days and are due and payable by the 15th day of the month in which you receive your statement.  Charges will be considered past due if payment is not received by the end of the month.  All late payments are susceptible to a $10.00 charge on your next statement.


Our Guarantee:

Every appliance constructed by Smile Keepers, Inc. comes with an unconditional guarantee  to fit the model we received.  We recommend that the doctor check all outgoing models and bite registrations for accuracy before sending them to the lab.  However, we will contact your office if there is a noticeable problem with a model or bite registration.  All appliances carry a 90 day guarantee from the time of delivery.

Repairs:

All breakages will be repaired or replaced free of charge if the breakage is the result of defective materials or workmanship.  The original and new models should be returned with the appliance so that a determination of the repair can be made.

Pick-up And delivery:

Local pick-up and delivery services are available in Tampa and the surrounding area.
